Required Documents for Lifeline: A Checklist
Save yourself a back-and-forth with support by gathering the right paperwork before you apply. Here's the full checklist.
Identity documents (required for everyone)
- A valid, government-issued photo ID — driver's license, state ID, passport, or military ID
- Proof of your current address if it's not listed on your ID (a recent utility bill or lease works)
If qualifying by program participation
- A current benefit award letter, statement of benefits, or approval notice
- Any document showing the issuing agency name, your name, and the date
If qualifying by income
- Prior year's state, federal, or Tribal tax return
- Three consecutive months of pay stubs
- A current unemployment or veterans benefits statement, or Social Security/retirement benefit letter
Tips for faster approval
Make sure documents are current (most benefit letters are only valid for a limited time), and that the name on your documents matches your application exactly. Blurry photos of paperwork are the single most common cause of processing delays.
